1. WHO IS ASKING FOR CONSENT

Before consent, HZ360 may collect only the child’s name and the parent’s name and online contact information to request consent. If consent is not received within the disclosed invitation period, HZ360 deletes the invitation information. A non-child-specific invitation code may route the request without collecting team affiliation before consent.
3. INFORMATION HZ360 REQUESTS AFTER CONSENT
- Account information: child’s name, age or birth date, parent contact details, credentials, and security information.
- Athlete profile: sport, position, team or club, graduation year, training activity, assessments, performance statistics, goals, and evaluations.
- Academic and recruiting information supplied by the parent or child.
- Communications and support information.
- Technical information required for authentication, security, and permitted internal operations.
- Only if separately authorized: wearable or motion data; photographs, video, or audio; and profile visibility to selected clubs, coaches, programs, or share-link recipients.
4. WHY HZ360 USES THE INFORMATION
HZ360 uses the information to provide the athlete profile, training and development features, requested performance analysis, safety and security, parent communications, consent administration, and parent-authorized recruiting or sharing features. HZ360 does not condition core participation on providing more information than is reasonably necessary for the requested activity.
5. ARTIFICIAL INTELLIGENCE
HZ360 may use automated analysis to create performance insights, reports, TacticalDNA™ indicators, recommendations, or derived scores. HZ360 will not send identifiable information about your minor child to an externally hosted AI model. An external AI provider may receive only information that has passed HZ360’s approved de-identification controls and is contractually restricted from reidentification, secondary use, or model training. Automated outputs are decision-support tools and may be challenged or corrected.
6. WHO MAY RECEIVE INFORMATION
- Approved hosting, database, authentication, security, communications, storage, and support providers acting only on HZ360’s instructions.
- HZ360 does not send your child’s personal information to the wearable provider. When you authorize the wearable feature, the provider issues HZ360 a secure API credential and HZ360 receives your child’s device data from the provider.
- Facilities, trainers, clubs, coaches, college programs, or other recipients only to the extent authorized for the child and program.
- Legal, safety, or government recipients when required by law or reasonably necessary to protect rights and safety.
HZ360 does not sell children’s personal information, use it for targeted or behavioral advertising, or make child profiles publicly searchable. Any additional operator that collects or maintains children’s personal information through the applicable Service is identified, with contact information, at https://hz360.ai/privacy/operators.
7. OPTIONAL CHOICES
The following choices are separate from base consent and are not selected by default. Refusing one disables only that optional feature:
- Wearable or motion-data collection, received by HZ360 from the approved provider through a parent-authorized connection. HZ360 does not send your child’s information to the provider.
- Photograph, video, and associated audio upload and analysis.
- Visibility of selected information to an approved club, coach, facility, or college program.
- Parent-authorized, limited, expiring, and revocable share links.
8. RETENTION, SECURITY, AND PARENT RIGHTS
HZ360 retains child information only while reasonably necessary for the authorized purpose and under its approved retention schedule. Protected backups are isolated and expire through the backup cycle. HZ360 uses reasonable administrative, technical, and physical safeguards and requires applicable providers to protect and delete information.
You may review your child’s information, request correction or deletion, refuse further collection or use, withdraw base consent, or withdraw any optional choice independently. 9. YOUR DECISION
Please review the HZ360 Children’s Privacy Policy (https://hz360.ai/privacy/children) and General Privacy Policy (https://hz360.ai/privacy) before completing the Parent Consent Instrument at https://hz360.ai/privacy/parent-consent. Consent is provided by completing and signing the Parent Consent Instrument electronically; HZ360 verifies that the person consenting is the child’s parent or legal guardian through the signed consent form completed and returned electronically with an identity check; payment-card verification is used where escalated verification is needed. If you do not provide base consent, HZ360 will not activate the child profile or collect additional personal information, and will delete the limited invitation information according to its retention schedule.
