4. REQUIRED BASE CONSENT

☐ I CONSENT to HZ360 collecting, using, and maintaining the child’s account, profile, athletic and development information, communications, technical information, and other core information described in the Direct Notice and Children’s Privacy Policy to provide the requested HZ360 services.

Base consent includes approved providers necessary for hosting, authentication, security, storage, communications, support, and core service delivery. It does not include the optional activities below. If base consent is refused or later withdrawn, HZ360 cannot maintain the minor athlete’s profile and will begin deletion under its retention schedule. This form may be used for any minor athlete; for athletes ages 13–17 it records the parent or guardian consent required by applicable law or program rules.

A. Wearable or motion data

6. AI AND AUTOMATED OUTPUTS

I understand that HZ360 may generate automated performance insights, TacticalDNA™ indicators, recommendations, or derived scores. HZ360 will not transmit identifiable information about my minor child to an externally hosted AI model. I may challenge or request correction of a material output. These outputs do not replace qualified coaching, medical, educational, legal, or recruiting judgment.

7. PARENT RIGHTS AND WITHDRAWAL

I understand that I may review the child’s information, request correction or deletion, refuse further collection or use, withdraw base consent, or withdraw an optional choice without withdrawing the others. Requests may be sent to legal@hz360.ai, made by telephone at 888-434-9360, or mailed to 1500 John F. Kennedy Blvd, Suite 450, PMB 101, Philadelphia, PA 19102. Withdrawal does not affect processing lawfully completed before HZ360 receives it.

HZ360 records each grant, refusal, withdrawal, notice version, verification method, and effective date. Temporary verification evidence is used only for verification, protected, and deleted under the approved retention schedule. Government identification must not be collected from a child. Any escalated parent-ID method requires prior approval by the Company’s Legal Officer and provider safeguards. A submission with conflicting or missing selections for any choice is treated as a refusal of that choice until the parent clarifies it.
